Job Summary
This role serves as a senior clinical leader of a financially at-risk value based medical group, responsible for overseeing all medical and behavioral health providers; ensuring the delivery of high-quality, cost-effective, and patient-centered care. This role is pivotal in aligning provider influenced clinical operations with value-based care objectives, improving health outcomes, and managing total cost of care.
Duties and Responsibilities
Clinical Leadership & Oversight
- Provides strategic direction and clinical oversight to all physicians, advanced practice providers, and clinical staff. - Ensure adherence to AbsoluteCare clinical pathways, evidence-based clinical guidelines and best practices. - Lead and set the culture of accountability for provider performance evaluations and review processes.
Quality & Outcomes Management
- Help develop and implement quality improvement initiatives to enhance patient outcomes and satisfaction. - Monitor and report on key performance indicators (KPIs) including cost of care, HEDIS, STAR ratings, and clinical documentation excellence risk adjustment metrics. - Drive initiatives to reduce unnecessary cost (referrals, testing), hospital admissions, readmissions, and emergency department utilization.
Utilization & Cost Management
- Collaborate with finance and operations teams to manage medical cost trends, operational overhead cost trends, productivity, and ensure financial sustainability. - Oversee provider practice patterns and how this directly impacts utilization management, care coordination, population health strategies, and productivity. - Support appropriate accurate documentation and coding opportunities.
Clinical Pathway Adherence
- Ensure adherence to clinical pathways and protocols to standardize care delivery that directly impact clinical outcomes and total cost of care (affordability). - Monitor and evaluate clinical pathway outcomes and make necessary adjustments.
Provider Recruitment, Retention, Engagement & Development
- Continue to build a highly engaged provider group
- Address provider concerns and support professional growth, satisfaction, and retention.
-Foster a culture of accountability, collaboration, and continuous improvement among providers. - Lead educational sessions, case reviews, and clinical forums to support provider development.
Population Health & Utilization Management
- Develop and implement population health management strategies to achieve cost savings goals. - Monitor population health, utilization metrics and outcomes to drive continuous improvement.
Patient Engagement
- Develop strategies to enhance patient engagement and satisfaction. - Implement patient-centered care models and shared decision-making practices.
Compliance & Risk Management
- Ensure compliance with all regulatory, legal, and accreditation standards. - Participate in audits, corrective action plans, and risk mitigation strategies.
Minimum Qualifications
· Licensed physician in the state of practice. (MD or DO degree from an accredited institution)
· Board certified Internal Medicine, Family Practice or relevant specialty.
· Active and unrestricted medical license.
· Minimum 5 years (> 5 years preferred) of successful experience leading providers within a multi-state at-risk value based primary care medical group. Center based and community based preferred.
· Experience and successful track record with value-based care, managed care, or leading an at-risk provider organization.
· Strong understanding of providers influenced interventions that improve total cost of care, quality metrics and population health.
Preferred Skills:
· Proven ability to lead multidisciplinary teams and drive performance improvement.
· Excellent communication, negotiation, and interpersonal skills.
· Proficiency in data analysis, EMR systems, and clinical reporting.
· Experience in at-risk medical groups within Medicaid, Dual or ACOs.
· BLS certification required.
Working conditions
This job requires the combination of remote work in a home office setting, professional office settings, and comprehensive care center environments. This role requires a dedicated, quiet workspace with the ability to adhere to HIPAA and other privacy policies. A reliable and high-speed Wi-Fi connection or home internet is required to perform the essential functions of this role.
