Job Summary
This role will lead the development and management of strategic provider relationships that support AbsoluteCare’s CKD care model. This individual will serve as the primary point of contact for nephrologists, dialysis providers, primary care practices, hospitals and skilled nursing facilities across the Ohio market. The role will support provider education, onboarding, performance management, and the success of network partnerships that enhance care coordination and outcomes for CKD patients.
This role is a key member of our value-based organization. Through our Beyond Medicine™ model, we provide comprehensive, in-home and community-based primary care, behavioral health, and social services to high-risk Medicaid and Medicare members. Our CKD Program supports patients with chronic kidney disease through proactive, personalized care that delays disease progression, reduces avoidable utilization, and improves outcomes.
Duties and Responsibilities
Operational Leadership
- Collaborate across multiple internal departments for client onboarding and lifecycle management.
- Monitor and improve client KPIs, shared savings, and Human Resource metrics.
- Assess the operational performance of client(s) and make recommendations to improve performance to achieve expected outcomes.
- Align client model with AbsoluteCare’s operating model.
Growth & Optimization
- Work with Finance and Analytics to deliver ROI-driven insights.
- Partner with senior executives to enhance service value and meet objectives.
- Develop new products or service lines that align with the mission, vision, and values of AbsoluteCare.
Team Development
- Identify and develop high-performing administrators.
- Align internal and client stakeholders to achieve outcomes.
- Establish KPIs and dashboards.
Minimum Qualifications
Required:
- Master’s degree in business or healthcare administration is required.
- 10 years in client success, account management, or value-based care operations (10+ years in leadership; healthcare preferred).
- Proven experience in value-based care, population health, or managed care.
- Strong grasp of healthcare regulations and payer-provider dynamics.
- Excellent communication, negotiation, and executive presence.
- Willingness to travel 15-30%.
Working conditions
This job operates in a remote location from your home location and within a professional office environment. This role requires a dedicated, quiet workspace with the ability to adhere to HIPPA and other privacy policies. A reliable and high-speed Wi-Fi connection or home internet is required to perform the essential functions of this role. This role requires reliable transportation to commute back and forth; and routinely uses general office equipment.
