Job Summary
The role of CKD Service Line Leader will collaborate closely with the Medical Directors and various care teams to provide expertise in the development and implementation of care strategies for nephrology patients. As a dyad partner with the Nephrology Medical Director, the CKD Service Line Leader will lead the design, implementation, and oversight of AbsoluteCare’s Chronic Kidney Disease care management program. This role is responsible for ensuring high-quality, evidence-based, and cost-effective care for members with CKD, with a strong emphasis on early intervention, patient education, and coordination across multidisciplinary teams. The CKD Service Line Leader will work closely with nephrologists, primary care providers, and care teams to improve outcomes and reduce the total cost of care for this vulnerable population.

Duties and Responsibilities
- Develop and manage a comprehensive CKD care management program aligned with value-based care principles.
- Work collaboratively with local leaders and local care teams in both the PCP Pods, Community Providers and Community Care teams as well as other services such as Pharmacy, Central Services, etc.
- Collaborate with nephrologists, primary care providers, and interdisciplinary teams to coordinate care plans for members with CKD stages 3–5.
- Monitor clinical outcomes, utilization metrics, and quality indicators to assess program effectiveness and identify areas for improvement
- Lead initiatives to reduce progression to end-stage renal disease (ESRD) and avoid unnecessary hospitalizations, including but not limited to: ED Diversion, reduction in hospital admissions, relationship building with dialysis centers to reduce missed treatments, and developing programmatic support to educate members and teammates
- Educate patients and families on CKD management, lifestyle modifications, and treatment options.
- Ensure compliance with clinical guidelines, regulatory requirements, and organizational policies and ensure there is a systematic approach to SOP and workflow creation, version control and at least annual reviews for relevancy regarding processes.
- Serve as a subject matter expert on CKD within the organization, providing training and support to clinical staff.
- Partner with analytics and population health teams to identify high-risk members and track performance metrics.
- Participate in strategic planning and budgeting for CKD-related initiatives.
- Partner with Nephrology Medical Director to directly support the CKD care management program and report up through the Market President, Market Medical Director and other local leadership roles
Minimum Qualifications
- Bachelor’s degree in Nursing, Public Health, Healthcare Administration, or a related field (Master’s preferred).
- Current RN license or equivalent clinical credential preferred
- Minimum of 5 years of experience in chronic disease management, with at least 2 years focused on nephrology or CKD.
- Demonstrated experience in program development and care coordination in a value-based care setting.
- Strong knowledge of CKD clinical guidelines, treatment pathways, and patient education strategies.
- Excellent communication, leadership, and project management skills.
- Team player who focuses on excellent member care, creative problem solving and overall local leader capable of continuous improvement
- Proficiency in using data to drive clinical decision-making and quality improvement.
- Willingness to travel regularly (up to 50%)
Working conditions
This job operates in a remote location from your home location and within a professional office environment. This role requires a dedicated, quiet workspace with the ability to adhere to HIPPA and other privacy policies. A reliable and high-speed Wi-Fi connection or home internet is required to perform the essential functions of this role. This role requires reliable transportation to commute back and forth; and routinely uses general office equipment.
