Specific Duties and Responsibilities

Complex Clinical Care:

  • Manages a tight panel of 200-300 members with focus on improving care and decreasing unnecessary and unhelpful utilization of care.
  • Fluid leadership of a talented team (case management, behavioral health, pharmacy, nursing) creating enhanced care plans that address a member’s medical and social factors.
  • Assertively manages transitions of care for members, especially immediately after discharge. 
  • Communicates and interacts with a member’s current care providers, coordinating and directing care.
  • Practices enhanced primary care, with treatment of acute conditions in the Center.  Consults specialists judiciously when needed. 
  • Uses a data-driven approach to manage the assigned panel of complex-care members.
  • Manages the electronic medical record to enhance a member’s care.
  • Values quality and proper utilization over volume.

 Candidate Qualifications

  • Unrestricted Pennsylvania medical license 
  • Board certified in Internal Medicine or Family Medicine
  • BLS certification
  • Special interest in service to a medically and socially complex population
  • Inclusive and open
  • Collaborative, team-driven, interested in utilizing the talents of a highly-skilled interdisciplinary team.
  • Interest in maximizing data, knowledge, and relationships to help those in need.
  • Independently driven to excellence. 
  • Strong attention to quality and value in healthcare.

Preferred qualifications: 

  • Experience in a team-based model of care (PCMH, FQHC, Community Health Center)
  • Leadership demonstration (Team captain, Chief resident, charge nurse, etc.!)
