Job Summary

 

The Treatment and Triage Paramedic (“TTP”) is a dynamic support role whose care environment encompasses both the center, community and hospital settings.

 

When working in the center, the TTP will triage and develop plans around urgent patient needs collaboratively with the Medical Director and Providers. They will perform ED follow up calls post discharge, and proactively leverage ABC resources to ensure care excellence, prevent readmission and ED utilization. They will assist in chart preparation, specifically establishing past medical history and current medication list for members.

 

In the community setting, the TTP will coordinate with the Community Based Provider (“CBP”) as they care for patient in their homes and other community locations. Together, they may perform hospital follow-up visits for patients at risk for readmission with emphasis on ensuring a smooth transition. This includes evaluating and updating the discharge plan, defining past medical history, medication reconciliation, and providing patient, family, and caregiver education.

 

As well, a medical screening process, triaging further community care needs may be performed in conjunction with Medical Director and Providers.

 

In the hospital setting, the TTP will perform the same chart preparation process on our unassigned members to establish medical continuity.

 

Clinical support by the TTP includes measuring vital signs, administering approved medication, phlebotomy, intravenous access, scheduling, managing DME requests and diagnostic imaging, as well as other administrative needs.

 

Duties and Responsibilities

 

  • Supporting Medical Providers in facilitating high quality primary, transitional, and acute care that promotes appropriate healthcare utilization.
  • Assisting CBP in at-home care requirements.
  • Triage community members additional care needs in conjunction with Medical Director and providers.
  • Managing patients in the Immediate Care Area.
  • Assist in Rapid Response procedure in all settings
  • Urinary catheter insertion and removal.
  • Respiratory treatments.
  • Perform phlebotomy and collection of other lab specimens.
  • Establish intravenous access
  • Administer fluid therapy and approved medications
  • Patient education related to self-administration of home antibiotic treatments.
  • Clinical assessments and help facilitating interventions from patient social needs to acute medical conditions.
  • Prepare and administer medications and vaccines with Provider authorization.
  • Drive to and from the medical center and member home/community.
  • Manage CBP schedule, and coordinate follow up appointments.
  • Prepare members for in person and telehealth examination as needed.
  • Take vitals, perform appropriate social, behavioral, and medical age and condition-based screenings.
  • Close quality measure gaps in all settings.
  • Proactively prepare new patient charts.
  • Initiate medication reconciliation.
  • Assist the provider with exams and minor procedures.
  • Review the after-visit summary with the member and ensure understanding through communication tools like teach-back and closed-loop feedback.
  • Ensure follow-up visits are scheduled prior to visit completion.
  • Follow protocols for infection control, including proper cleaning and disinfection of medical equipment.
  • Perform pre-visit planning to ensure high quality provider visits.
  • Coordinate services to complete CBP action plan tasks.
  • Provide coverage support for fellow care team members onsite as needed.
  • Leverage strong time management skills to prioritize and balance member care responsibilities with role-specific tasks

 

Minimum Qualifications

  • High School diploma
  • Licensed Paramedic in state of practice.
  • 3 years of experience
  • Excellent IV skills and experience required.
  • Current ACLS/BLS Certification
  • Phlebotomy Skill
  • Understand and abide by OSHA, HIPAA, and CLIA standards.
  • Excellent written and verbal communication skills.
  • Great documentation habits; able to quickly learn a new EMR system.
  • Proficiency with Word, Excel, Outlook, and PowerPoint.
  • Personal motor vehicle preferred.
  • Active driver’s license and proof of insurance in state of practice preferred.

 

 

Working conditions

 

This job operates in the community and within a professional office environment. Community settings can vary and include patient homes, homeless shelters, and shared public spaces.  This role routinely uses general office equipment and often wear a special uniform, such as a lab coat or surgical scrubs, protective or safety attire, such as gloves and masks. Exposed to the diseases and infections of patients daily. May share workspace with other technicians and staff.

  • Ability to communicate clearly and exchange accurate information constantly.
  • Ability to remain stationary for long periods of time.
  • Constantly operates computer, keyboard, copy and fax machine, phone, and other general office equipment.
  • Operates personal motor vehicle.
  • Ability to occasionally move objects up to 20 lbs.
