Job Summary

 

This role will be a community-based provider who triages calls we receive from our members after clinic hours regarding medical and behavioral health complaints. This role will assess the situation and decide if that patient can stay at home and be seen in the clinic the next day or requires urgent or emergent care.  This role will have access to technology if a face to face is required with an advanced practice provider (NP) or a physician.  Paired with another member of our team and will be based out of our clinic after hours. Additionally, this role will do post discharge checkups on our patients including medication reconciliation and helping with any needs our patients may have.

 

Duties and Responsibilities

 

  • Triage calls made to the clinic after hours through in person assessments.
  • Discuss case with on call provider.
  • Use critical decision-making skills to decide if patient needs immediate medical attention.
  • Protocolized treatment of various low acuity complaints.
  • IV therapy; IV and oral medications.
  • Use interpersonal skills to go out in the community and meet the patient where they are.
  • Help patients with whatever needs they may have after hours
  • Arrange for close follow up (next day) with appropriate clinic-based resource (social work, behavioral health, PCP visit, pharmacy, radiology, lab).

 

Minimum Qualifications

 

  • Nationally Registered Paramedic in the State of practice.
  • ACLS, BLS.
  • Mission driven to serve the underserved.
  • Strong focus on customer service.
  • Ability to thoughtfully build successful relationships with team members and our partners.
  • Experience in working with psycho-socially and medically complex patients is strongly preferred.
  • Must be able to work as part of a multidisciplinary team with constant collaboration within and across provider teams.
  • Experience and comfort utilizing EMR.
  • Experience using telehealth resources.
  • Strong communication skills.
  • Creative problem solver.

 

Working conditions

 

This job operates in a professional office environment.  This role routinely uses general office equipment.  Often wear a special uniform, such as a lab coat or surgical scrubs, protective or safety attire, such as gloves and masks. Exposed to the diseases and infections of patients daily. May share workspace with other technicians and staff.  Also operates out in the community served.

 

Physical requirements

 

  • Ability to communicate clearly and exchange accurate information constantly.
  • Ability to remain stationary for long periods of time.
  • Job may be physically demanding at times.
  • Repetitious movements.
  • Constantly operates computer, keyboard, copy and fax machine, phone, and other general office equipment.
  • Ability to occasionally move objects up to 20 lbs.
