AbsoluteCare, Atlanta, established in 2000, is a NCQA certified Patient Centered Medical Home.  AbsoluteCare provides wrap-around full-service care to 3000 people living with HIV and another 3000 in the primary care division. AbsoluteCare provides on site social work services, integrated behavioral health, nutrition counseling, health education and on-site pharmacy with delivery services.  AbsoluteCare has been designated a HIV Center of Excellence by Georgia Medicaid and is considered a Transgender Center of Excellence for its work in gender affirming care.  It has achieved Top Performer Designation in the Human Rights Health Equity Index

Specific Duties and Responsibilities

  • Provide care for people living with HIV  and associated medical conditions and patients with other chronic infectious diseases.
  • Provide outpatient care consultation and ongoing treatment of chronic infectious diseases.
  • Deliver comprehensive care for patient panel, including management of chronic diseases directly or through collaboration with primary care physicians and specialist.
  • Work in a team environment (with social work, care management, integrated behavioral health, pharmacy, nutrition, health education etc.), process feedback, and facilitate open discussion.
  • Implement plans of care in collaboration with the patient, their family and other team members.
  • Develop strong transitional care plans and able to keep track of paneled patients along the whole continuum of care including hospital discharges, ER, skilled nursing facilities and home care.
  • Vigilant in keeping track of patients and their disease processes to avoid ER visits, admissions to the hospital and readmissions.

 Candidate Qualifications

  • Licensed Physician in the state of practice
  • Certified NP or PA licensed in the state of practice with American Academy of HIV Medicine Certification (AAHIVM) or Board certified in Infectious Diseases
  • BLS certification required
  • Experience in population health management preferred
  • Ability to work as part of a multidisciplinary team with constant collaboration within and across provider teams
  • Experience and comfort utilizing EMR
  • Problem-solving and the ability to be creative in those solutions
