Sign on Bonus: Up to $5000 (amount based on experience)

Hybrid work, 2 days at center, 3 days remote/community, Monday through Friday, 8-5 with hour lunch, no weekends, no on-call, with paid holiday time off and paid CEU.

The Community Nurse Care Manager (CNCM) completes a comprehensive assessment and creates a person-centered care plan that identifies and prioritizes health care goals with the member. The CNCM supports members meeting their goals through clinical interventions including patient education with teach back, self-management coaching, motivational interviewing, harm reduction strategies, and trauma informed approaches. The CNCM coordinates the services with their community primary care providers and specialists in addition to AbsoluteCare providers, required to meet the member’s individual needs. The CNCM works closely with the ICT to support our approach to holistic care management. This entails providing nurse consultations and receiving support from interdisciplinary colleagues including transitional care managers, behavioral health care managers, behavioral health consultants, community based and immediate care nurse practitioners, resource specialists and community health workers to holistically address all components of care. The CNCM meets members where they are with the flexibility to conduct member visits telephonically, in the home, community and/or office setting.

Duties:

Serve as the primary care manager for members identified as having medical or functional health conditions as the primary driver of health care utilization Provide holistic care management and person-centered care planning for members with chronic, complex needs Meet with member’s community primary care providers and/or specialists driving member care and collaborate to address member’s whole person needs medical, behavioral and social determinants of health.

Education and Experience:

  • Registered Nurse with 3 + years related case management experience, with 3 + years of diverse clinical background in >2 care settings (hospital, SNF, home health, rehab etc.);
  • Multiple settings experience (Inpatient, SNF and home health) preferred.
  • Bachelors with case management certification preferred;
  • Experience with eClinicalWorks preferred;
  • Active unencumbered driver’s license, with automobile insurance, reliable transportation, and ability to work in office and in the community.
