Monday through Friday, 8-5 with hour lunch, no weekends, on-call, or major holidays

The Social Work Care Manager is a core member of the collaborative integrated care team.  This role provides, to an assigned caseload, a range of community-based client-centered clinical interventions and clinical support activities, including conducting biopsychosocial intakes, assessments, and screens, care plan development, implementation, evaluation, and modification, supportive counseling, crisis intervention, referral to support services, and clinical consultation.  This position is centered in a trauma informed care approach with emphasis on harm reduction, motivational interviewing, problem solving and acceptance & commitment therapeutic techniques.  This role functions as a part of a large integrated care team with the goal of improved member health, decreased inappropriate medical utilization, and long-term member engagement.  This role will transition from a panel-wide generalist approach to a caseload-based position emphasizing clinical interventions over the course of the panel’s growth.  

As a Social Worker Care Manager, this role:

  • Provides the opportunity to act as a Field Instructor for the MSW program. 
  • Conducts comprehensive biopsychosocial assessments for a diverse patient population. 
  • Complete psychosocial section of initial assessments within 30 days of assignment and conduct mandated outreach to facilitate completion of the initial assessment. 
  • Develop, coordinate, and implement a client-centered multidisciplinary care plan that establishes SMART goals and includes risk reduction strategies as needed. 
  • This position may conduct member visits in the community and/or office setting, based on the member's needs.  

Education & Experience:

  • Master’s degree in Social Work
  • LMSW required, Licensed Clinical Social Worker in the state preferred
  • 3 years experiences in serving the needs of complex populations, including medically complex, trauma history, mental health conditions, substance abuse, and socioeconomic barriers in an office or community-based setting.
  • Knowledge of managed care and insurance processes is preferred.
  • Experience with the management of Medicare, Medicaid, and commercial populations.
  • Reliable transportation and ability to work in office or in the community
